§ “People who need to attend the examination this time(who receive an announcement or E-MAIL) must comply with the description below”

Employees should do the health examination regularly according to the regulations, and turn in a formal report copy to the “Office of Environment and Safety ”(People who didn’t finish the process, would receive a penalty)

1.Finish the health-examination before 2021/11/30. 

2.Select a qualified medical institution: 

   (1). Physical examination at school on November 10(Chi-shin Clinic)

   (2). Please have a health check at recognized medical institutions; relevant information is available at https://hrpts.osha.gov.tw/asshp/hrpm1055.aspx

3.Submit the health examination report to the “Office of Environment and Safety ”

   (1)The submitted information includes: 

• Health-examination report :Annexe 1 
• The Physical and Mental Health Questionnaire of the Family Medicine Department : Annexe 2 
• Nordic. Musculoskeletal Questionnaire : Annexe 3 
• Overwork Survey :Annexe 4

   (2)Submission Methods of health examination report (Choose one):

• Please scan the formal report, and email the scan file to Ms. Kuo(E-mail:G10926@mail.ntust.edu.tw)
• Please turn in the copy of the report to the “Office of Environment and Safety” at IB - floor 11 in working hours.

• If you attend the “in-school” examination, the report will be provided by the cooperating clinic, you don’t need to hand out the document in person.
• If  you already has been carried out health examination,submit report in the time span according to age, please.
