• The new employees physical examination report must meet the following requirements

1. Please go to: https://hrpts.osha.gov.tw/asshp/hrpm1055.aspx to search for medical institution approved by the Ministry of Health and Welfare to complete your new staff health exam.

(please ensure that it is stamped with the physical examination medical entity’s agency stamp.)

2. The employers shall conduct pre-employment physical examinations for laborers at the time of employment; for currently employed laborers, the following health examinations shall be conducted:

          (1)Special Physical Examination(for Specially hazardous workers )

•  The examination items:  Please see attached for more details and bring this form to athe designed hospitals for the health examination.:
•  NTUST accepts Special Physical Examination reports that are within one year before the date of employment.

     (2)General Physical Examination (ALL New Employees other than Special Hazardous Working)

• Examination Items: (The following items are mandated inspections,not one less.)
• Chest X-ray:

1. Please inform the doctor if you are pregnant. (You are allowed to skip the CXR exam when you are pregnant.)
2. If there is any abnormality, please take the initiative to re-check.

• NTUST accepts General Physical Examination reports that are within the validity period before the date of employment.
