｛General Physical Examination Items｝

Required to comply with regulatory health examination items: Attachment 11: General Physical and Health Examination Records of Workers

1、 Individuals over 65 years old should undergo an annual check-up.

2、 Individuals over 40 years old but under 65 years old should undergo a check-up every three years.

3、 Individuals under the age of 40 should undergo a check-up every five years.

The general health examination items and examination records specified in the preceding paragraph shall be handled in accordance with the provisions of Annex 9 and Annex 11 of the preceding article. However, those who have been diagnosed with congenital abnormalities in colour discrimination may be exempted from further colour discrimination tests.

 

｛Special Physical Examination Items｝

Required to comply with regulatory health examination items: Appendix 10 Special Physical Examination Items Table

According to the controlled chemicals registered online in each laboratory, if they contain the 32 job categories listed in the above table, a special health examination questionnaire for labour can be filled out, reviewed, and stamped by the unit supervisor.

 

｛Health Examination Institute｝

According to the regulations of the Ministry of Labor on the recognition and management of medical institutions for labour physical examination and health examination, it is necessary to obtain medical institutions recognition for labour physical examination and health examination

｛Explanation of Health Examination Subsidy Costs｝

> 2025 List of General Health Examination (Attachment)

> 2025 General Health Examination Subsidy Cost NTD1250 (Subsidy amount may differ every year)

 

(1) Civil servants and educational personnel

> Those who qualified for ‘central insitution employees general health examination subsidy’ (refer to personnel who reach the age of 40 as of December 31 of the previous year):Subsidy NTD 4500 for official health check every 2 years.

> Not qualified for above saying: Subsidy NTD1250 for 2025 general health examination.

> Expense redeem method:

1. Campus physical examination: fees pay on-site, redeem subsidy from counter of Personnel Office on-site with reciept.

2. Off-campus physical examination: 1 day-off given with official application for examination. The fees shall be paid in advance. Provide examination report and receipt to Personnel Office afterwards to redeem subsidy.

3. If personnel is retired, subsidy of NTD4500 is not applicable.

(2) Full-time assistant and  Post-doc researcher

> Subsidy NTD1250 for 2025 general health examination

> Expense redeem method: research project personnel are supported by the budget complied by project host. (On-site payment is still required in advance, submit application afterwards with notification letter from Office of Environment and Safety and receipt of the examination, example as shown below)

1. Campus physical examination: fees pay on-site, submit application afterwards to redeem.

2. Off-campus physical examination: 1 day-off given with official applpication for examination. The fees shall be paid on-site. Provide examination report to Office of Environment and Safety and submit application to redeem.

 

(3) Contract workers, project lecturer and part-time lecturer

> Subsidy NTD1250 for 2025 general health examination

> Expense redeem method:

1. Campus physical examination: Select general package with a cost of NTD1250, no payment require. (If any optional checkup added, on-site payment is required)

2. Off-campus physical examination, 1 day-off given with official application for examination, fees shall be paid on-site, provide examination report and receipt to Office of Environment and Safety to redeem subsidy.

 

(4) Other faculty and new employees who do not meet the above conditions may participate in the "on campus" health examination activities at their own expense.

Payment Method - According to the selection plan, payment will be made on the same day upon on-site check-in.

Please submit a physical examination report in accordance with Article 20 of the Occupational Safety and Health Law; Violators shall be fined up to 3000 yuan in accordance with Article 46.

Please submit the physical examination report as soon as possible to maintain your own health and fulfil legal labour obligations. Those who fail to submit within the deadline will notify the unit supervisor.
