｛General Physical Examination Items｝

Required to comply with regulatory health examination items: Attachment 11: General Physical and Health Examination Records of Workers

1、 Individuals over 65 years old should undergo an annual check-up.

2、 Individuals over 40 years old but under 65 years old should undergo a check-up every three years.

3、 Individuals under the age of 40 should undergo a check-up every five years.

The general health examination items and examination records specified in the preceding paragraph shall be handled in accordance with the provisions of Annex 9 and Annex 11 of the preceding article. However, those who have been diagnosed with congenital abnormalities in colour discrimination may be exempted from further colour discrimination tests.

According to the controlled chemicals registered online in each laboratory, if they contain the 32 job categories listed in the above table, a special health examination questionnaire for labour can be filled out, reviewed, and stamped by the unit supervisor.

｛Explanation of Health Examination Subsidy Costs｝

Refer to the list of general health examination civil servants/environmental safety room expenses/planned subsidies for the year 2023, as attached.

If the planning personnel are unable to cover the medical examination fees due to factors such as the Ministry of Education, please notify the Environmental Safety Office to use the Environmental Safety Office funds instead.

（1） Official health check every two years, with a benefit of 4500ntd

• Eligible: Those over 40 years old who belong to the public and educational personnel insurance (referring to those who have reached the age of 40 as of December 31 of the previous year)
• Expense verification method

1. Those who participate in the campus physical examination activities will pay the fee on-site on the same day, present the receipt, and apply for verification with the personnel in charge of the human resources department on site. If you have any doubts, please contact Miss Zheng 1047 in the human resources department directly#
2. For those who undergo off campus physical examinations, the examinee shall first pay the physical examination fee on their own, provide the physical examination materials and receipts to the personnel office for verification. For details, please contact Miss Zheng 1047 of the personnel office directly#
3. If a civil servant has retired, the subsidy of 4500 yuan is not applicable. If there is any doubt, please contact Miss Zheng in the Personnel Department (1047 #) (For technicians and colleagues, please contact Miss Xiong in the Affairs Department (6158 #)

（2）General health examination subsidy for the year 2023 is 1100 yuan (The subsidy amount may differ every year)

• Eligible: Those who belong to labour insurance/those under the age of 40 who are insured by public education personnel
• Expense verification method

1. For those who participate in the 112 year campus health check up activity, no fee will be charged within NT $1100. (If there are self funded or excess projects, please make up the difference on site).
2. You can take one day off from work with official documents and go for a physical examination outside of school. If you go to Chi Hsing Clinic or other medical institutions for a physical examination on your own, the examinee will first pay for the physical examination fee and provide a physical examination report and receipt to the environmental safety room for verification.

（3） Subsidy for research project personnel (only available for those who are eligible for medical examination this year)

Research project personnel (such as full-time assistants and postdoctoral researchers) are all supported by the budget compiled by the project host (on-site payment is required on the same day, and then submitted with a notification letter from the Environmental Safety Office and signed by oneself. The example of signing is as follows).

（4） Other faculty and new employees who do not meet the above conditions may participate in the "on campus" health examination activities at their own expense.

Payment Method - According to the selection plan, payment will be made on the same day upon on-site check-in.

Please submit a physical examination report in accordance with Article 20 of the Occupational Safety and Health Law; Violators shall be fined up to 3000 yuan in accordance with Article 46.
