實驗室安全衛生概論
Introduction of Safety and Hygiene Management in Labs

※課程對象Teaching object

1. 各大專校院需進入實驗場所之新進人員(新進之碩博士學生、大學專題研究學生、研究人員等)。
2. 依職安法規定，實驗室新進人員須接受3小時一般安全衛生教育訓練，若會使用化學品之新進人員請額外接受3小時化學品危害通識課程。
a. The members including new bachelor, master or PhD students and researchers who will access to the laboratory must attend this course.
b. According to the regulations of Occupational Safety and Health Act., the new laboratory workers need to complete another 3-hour chemical hazard awareness course.

Enter the “Management system for laboratory safety health, and environment” (http://140.118.70.185/Front/Login.aspx) to fill out the registration form. (As detailed in attachment I)

1. 本次課程僅提供茶水不提供餐點，為響應環保，請參加學員自備環保杯(現場一律不提供免洗餐具)。
This course only provides tea, no meals. Please bring your own mug.
2. 請利用臺科大環安管理系統線上報名，學員經測驗合格後將以本校環安衛系統方式發給電子證書，經取得證書者方可進入本校實驗(習)場所工作或研究。
Please register online via “Management system for laboratory safety health, and environment”. Once you have passed the test, a certificate will be issued to you. Only the person who has a certificate is allowed to enter the labs.
3. 考試成績請上報名系統查詢，成績達70分以上為合格，本校不及格者請與環安室連絡預約補訓且不得進入實驗場所，外校不及格者請另找課程訓練。Please completing the relevant trainings before starting to work on NTUST campus.
Test results are available on “Management system for laboratory safety health, and environment”. The passing score is 70, the person who failed the test should make an appointment for further training with our office.
4. 各梯次報名截止時間及名額限制詳見表1。

本次實驗室安全衛生概論教育訓練共辦理5場次，上課時間、地點詳如表1，訓練議程如表2~6所示。

2. 登入 Log in
(1) 本校人員：請直接點選”校內帳號登入(校內人員)”登入。(帳號為學號或本校e-mail帳號，密碼為本校e-mail密碼。) For NTUST's Students and staffs：Please click the button ”Login (Students and Staffs)” to log in. (Account：Student ID or E-mail ID，Password：E-mail password)

(2) 他校或無法以本校帳號登入者：至”校內/外人士帳號申請”完成帳號申請審核通過後→自”其他帳號登入(校內/外人士)”登入。For others (or if you can't log in)：Please click the button “A/C App for Others” to apply a new account. Log in from the button ”Login (Others)”.

(3) 請選擇申請類型：選擇本校或非本校人員
Choose the account type：Are you the NTUST’s students and staffs or not.

(4) 填寫申請資料後按”申請(Apply)”，待審核後即可登入，進入教育訓練報名區報名。Fill out the form and click the button below, then you can register the training course when you pass the application.
