※教育訓練證明

一般安全衛生及化學品危害通識教育訓練，訓練證明將於完成訓練兩週內，以e-mail寄送訓練證明，如有未收到訓練證明請來信amandayu@ntust.edu.tw或來電環安室#1232查詢。

※注意事項Registration Information and Notes

• 請進行線上報名，學員經測驗合格後將由環安室寄送電子證書，經取得證書者方可進入本校實驗(習)場所工作或研究。 

Please register on-line . When you have passed the test, we will mail you the certificate by Environmental and Safety Office. The certificate allows the person to enter the lab.  

• 考試成績結果成績達70分以上為合格將由環安室寄發訓練證明，成績不及格者不得進入實驗場所，環安室將通知補訓，外校不及格者請另找課程訓練。

If the quiz result is 70 points or more, the training certificate will be sent e-mail by OES. Those who fail the quiz will not be allowed to enter the laboratory， OES will notify make up quiz or training again . Those who fail in other schools should find another course training.

• 本次實驗室安全衛生概論教育訓練共辦理5場次，上課時間、地點詳如表1，訓練議程如表2~6所示。
