Application Procedure for Biological Laboratory (BSL‑2 Infectious Biological Operations)
Note: For BSL‑1, no separate application form is required. Simply select “BSL‑1” or “Chemical / BSL‑1” in the Environmental Health and Safety (EHS) system.
Please refer to relevant informations from the Ministry of Health and Welfare Taiwan Centers for Disease Control laboratory biosafety inspection (updated May 24, 2024).
Application Steps and Required Documents
- Submit an application through the (Environmental Safety and Health System) EHS system, selecting laboratory type “BSL‑2” as the laboratory type (Application website for newly established laboratories). Note: For an existing registered laboratory adding biological experiments, only complete and email the documents listed below according to the biological materials used; no additional online system application is required.
- Complete the following three documents and send them to the Environmental Safety and Health Office administrator's email address (amandayu@mail.ntust.edu.tw) for review:
- (1) Basic Information Form for Biological Laboratory (equipment / personnel / general information) – ODS file.
- (2) BSL‑2 Biological Laboratory Self‑Assessment Form – ODT file.
- (3) RG‑2 Biological Material Storage Site Self‑Assessment Form – ODT/ODS file.
After the EHS Office and the NTUST's Biosafety Committee have reviewed and confirmed the documents, laboratory online portal authorization will be granted.
Additional Notes
- Those borrowing laboratories must also update their laboratory location information in the system, as the Centers for Disease Control and Prevention (CDC) requires quarterly reporting of the remaining quantities of BSL‑2 or higher infectious biological materials. This process continues until the laboratory is no longer used.
- 2. The laboratory system currently does not have a function to request the purchase of infectious biological materials of level risk group 2 or above. Accordingly, for any request to purchase biological materials, you need to fill out application paper form and submit it to the Office of Environment and Safety and the University Biosafety Committee for review and approval. Please do not purchase them on your own, independent or unauthorized purchasing is strictly prohibited.
